- Respiratory arrest or failure
- Actual or potential airway obstruction
- Glasgow Coma Scale of 8 or less
- Need for hyperventilation (e.g., severe traumatic brain injury)
- Intrathoracic or other severe (multiple) injuries
- Decompensated shock
