Diagnose by:
- Index of suspicion: likelihood of blunt force trauma to abdomen, hemodynamic instability (low blood pressure, high heart rate)
- Changes on serial abdominal examinations
- Disrupted capsule, extravasation on abdominal CT scan
- Abnormal ultrasound (FAST) examination
- Blood, amylase or bilirubin on DPL
